You could go for a prescription-strength retinoid.

Just make sure to go easy on it. First, use it every other day and work your way up, if not your skin will flare up.

I used tretinoin for a while and my skin broke out so badly, it was literally peeling off, very bad!
Looking for better skin, I stopped it completely. You can try a non-prescription Retin-A that's not going to be so harsh on your skin.

It depends on your skin needs. For an acne treatment, for example, visiting a derm every 3-months is very common. For anti-aging usually, monthly visits for treatments are common. If it's just for check-ups, once or twice a year is enough.
