• Are you pregnant or nursing?
• Are you on medication? If so, what?
• Are you under a physician’s care? If so, for what?
• When was your last facial?
• Do you receive facials regularly?
• What is your skin care regimen?
• What is your goal today or desired result?
• How did you hear about us?
• Do you get fillers or anti-wrinkle injections?
• Have you received any peeling treatments? If so, when?
